1、中山-西渡教学模式开展的实践和体会 目的探讨中山-西渡教学模式开展8个月来的教学实践和体会,摸索适合在郊区社区开展的特色教学模式。方法从2018年5月开始,西渡社区卫生服务中心开展“中山-西渡教学模式”,共有19名医生参与,其中中心师资学员2名,每6个月轮换一次;其他学员包括规培生8名、新乡村医生4名、低年资医师5名,每3个月轮换一次。每周三上午由复旦大学中山医院副主任医师带教,学员参与,学员在学习期满时接受临床理论和接诊技能考核。同时,对2018年5月12月该模式教学接诊的233例患者进行满意度问卷调查,其中初诊患者156例。结果学员结束培训时,考核全部达到优良。学员经带教后临床思维缜密,均
2、能独立完成患者接诊和初步处理,初诊患者非常满意率达98%。结论中山-西渡教学模式能起到医患双赢的成效,可作为郊区社区卫生服务中心的教学模板。 教学模式 全科医生 郊区社区 G642 A 1006-1533(2018)08-0013-03 Practice and experience in carrying out the Zhongshan-Xidu teaching mode GU Xiaoqing1, GU Jie2, CHEN Haiying1, JIANG Hua1, YUAN Meijuan1 (1. Zhongshan Hospital affiliated to Fudan University, Shanghai 200032, China) ABSTRACT Objective To discuss the practice and experience in carrying out the Zhongshan-Xidu teaching mode for 8 months in order to explore the suitable teaching mode with charact
4、eristics of suburban communities. Methods From the beginning of May 2018, Xidu Community Health Service Center developed the Zhongshan-Xidu teaching mode and a total of 19 doctors participated. Among them 2 were teacher trainees with a rotation of 6 months and other trainees with a rotation of 3 mon
5、ths included 8 standardized training students, 4 new rural doctors and 5 junior doctors. During the training course, the associate chief physician from Zhongshan Hospital of Fudan University guided and taught the trainees how to manage the diseases every Wednesday morning .The trainees received the
6、clinical theory and treatment skill examinations at the end of the training. The questionnaire survey was conducted in 233 patients who were treated in this teaching mode from May to Dec. 2018, including 156 newly diagnosed patients. Results At the end of the training, all the students achieved the
7、excellent assessment in their careful clinical thinking and independently completed initial diagnosis and treatment. The satisfaction rate of the patients newly diagnosed and treated reached 98%. Conclusion Zhongshan-Xidu teaching mode can have the doctor-patient win-win results, and can be used as
8、a teaching template for the suburban community health service centers. KEY WORDS teaching mode; general practitioner; suburban community 从2018年起,上海将全科医师培训纳入全市住院医师规范化培训计划统筹考虑,全科医学作为住院医师规范化培训的27个培训学科之一,未经培训的人员不得从事全科岗位工作。如何培养和吸引一支高素质的全科医生队伍,让高素质的全科医生扎根在社区,是摆在每家郊区社区卫生服务中心面前的难题。全科医师的数量与质量已成为制约基层医疗机构服务能力的
9、关键因素1。中心于2018年11月成功创建为奉贤区唯一一家上海市住院医师规范化培训社区教学基地,同年12月与区中心医院“全科医学临床培训基地”共建签约,全面启动社区教学工作。 通过2年的带教实践,中心已逐步建立起教学体系及教学流程,师资也具有了一定的带教经验,中心的师资均由原内科、外科、儿科、妇科医师通过全科医师培训转岗而来,和部分其他社区培训基地的师资一样,对社区常见健康问题诊疗、重点人群健康宣教的工作经验非常丰富。但与优秀的兄弟单位相比,尚缺乏有效的教学方法,不能把知识系统全面地教授给学员2,难以培养出诊疗技术全面的全科医生。如何突破这一瓶颈,是急需解决的一个问题。为此,在奉贤区卫生和计划
